Useful Information

In order to receive copies of your medical records, please contact the Medical Records Department. So as to ensure the privacy and protection of personal data, the following apply:

If you are a patient, you can receive copies of you record upon your request, either in person by displaying your National Identity Card, or by courier, if you submit a request in person or send us a Solemn Declaration via gov.gr, where you will state the shipping address.
If you are an authorized third party, you can receive copies by displaying your National Identity Card and the patient's authorization, which must be validated by a public Authority or via gov.gr.
In cases of neonates or minors, requests and receipt of copies of medical records are made (a) by the parents, provided that both parents display their National Identity Cards; or (b) by one of the parents by displaying their National Identity Card and an authorization from the other parent validated by a public Authority or via gov.gr; or (c) by an authorized third party by displaying their National Identity Card and an authorization from both parents, which must be validated by a public Authority or via gov.gr.
